An OSCE exam is a practical test in which you rotate through a series of short, timed stations and are marked on what you actually do — take a history, examine a patient, perform a skill, explain a plan. Instead of one long case in front of one examiner, every candidate meets the same stations, the same simulated patients and the same mark sheet. This guide explains how an OSCE exam is built, how it is scored, what current pass-rate data really shows, and how to prepare for one without burning yourself out.

An OSCE exam is a clinical assessment made up of several short stations, each testing one defined task under the same conditions for every candidate. The acronym stands for objective structured clinical examination. "Objective" means the examiner marks against a written checklist rather than a general impression. "Structured" means the task, the time and the scenario are fixed in advance.
In practice you stand outside a door, read a short instruction ("This patient has had chest pain for two hours. Take a focused history and present your differential"), a bell rings, and you have a set number of minutes to do exactly that. Then the bell rings again and you move on. A station might use a simulated patient (a trained actor), a manikin, a piece of equipment, or a colleague playing a relative.
The format exists because the older style of clinical exam was unfair by design: one student got a straightforward patient with a clear murmur, the next got a complicated case and a tired examiner. Splitting the exam into many short, identical stations spreads that luck out. It also lets a school test things a single long case never could — hand hygiene, breaking bad news, safe injection technique, prescribing, consent.

Structure varies by institution, but almost every OSCE exam shares four building blocks:
The UK's nursing regulator publishes one of the clearest public breakdowns. The Nursing and Midwifery Council (NMC) uses the OSCE as part two of its Test of Competence for internationally educated nurses and midwives, and its own page describes a 10-station circuit: four stations linked together around a single patient scenario (assessment, planning, implementation and evaluation — "APIE"), four skills stations arranged as two pairs, and two stations assessing values, behaviours and evidence-based practice. The NMC states it moved from six stations to ten so the exam is "reflective of clinical practice".
Medical school OSCEs are built the same way but the details are set locally. Your faculty decides how many stations there are, how long each one lasts, whether stations are equally weighted and what counts as a pass. The UK licensing route for international doctors is a useful comparison point: as our PLAB exam guide sets out, PLAB 2 is an OSCE of 16 eight-minute stations plus rest stations. Same format, completely different rulebook.
Most OSCE exam mark sheets combine two things: a checklist of specific actions (introduced yourself, washed your hands, checked the patient's identity, offered analgesia) and a global judgement from the examiner about whether your performance was at the standard expected. Many schools then set the pass mark using a borderline method, where the cut-off is derived from how candidates rated as "borderline" actually scored, rather than being fixed at a round number in advance.
Two consequences matter for how you revise. First, marks are earned for doing things out loud and visibly — an examiner cannot tick "considered safeguarding" if you only thought about it. Second, a strong station rarely rescues a disastrous one, because many circuits require you to pass a minimum number of stations as well as reach an overall score.
The NMC's version is unusually transparent about what happens next. Candidates get their result by email within five working days, and anyone who fails receives feedback naming the stations and elements they did not pass — because on a resit they only repeat the stations they failed, not the whole circuit.
Real numbers are hard to find, because most universities never publish theirs. The NMC does, quarterly, and the pattern is striking. Between January and March 2026, 3,009 candidates sat the NMC OSCE and 78% passed — but that figure includes resits. Split by attempt, the picture changes completely.
Read that chart carefully, because it is the single most useful thing on this page. The gap between a 26–49% first-attempt rate and a 76–84% second-attempt rate is not a story about ability. It is a story about familiarity with the format. The same people, a few weeks later, with a clear picture of what the circuit feels like, pass at roughly double the rate. Every hour you spend rehearsing the mechanics of an OSCE exam before your first sitting is buying you part of that gap in advance.
The NMC itself points at two drivers of failure that have nothing to do with clinical knowledge: uneven access to good preparation and support, and anxiety — it notes that very anxious candidates make more errors, which in turn increases failures.
This is where most generic advice goes wrong, so be precise about your own situation. There is no international OSCE standard and no global authority that sets one. Broadly you will meet one of three types:

Two practical points follow. First, download your own institution's OSCE handbook and read the marking section before you read anything else — that document, not a blog, is the one that binds you. Second, check the logistics close to the date: they change. The NMC's OSCE is currently delivered by three approved test providers (University of Northampton, Ulster University and Leeds Teaching Hospitals NHS Trust) after the Oxford Brookes University centre closed on 31 July 2026 and the Northumbria University centre closed on 19 February 2026.
For the NMC route specifically, the published rules are worth knowing because they are unusually strict:

If you are following a different country's route, look for the equivalent published document from your own regulator. Do not assume the numbers above transfer — they are UK nursing and midwifery rules and nothing else.

Preparation for an OSCE exam is mostly rehearsal, not reading. Work through these steps in order:
If you have already failed a station, treat the feedback as a to-do list rather than a verdict. The pass-rate data above shows that this is exactly what most candidates do successfully.
It can, for one specific reason: volume of supervised patient contact. Stations reward candidates who have taken hundreds of histories, not candidates who have read about taking histories. A structured placement gives you daily repetitions with real patients and a supervisor who can correct you, which is the raw material an OSCE exam is testing.
A placement abroad adds something extra that transfers surprisingly well. Working in a hospital where you cannot rely on shared shorthand forces you to slow down, signpost, check understanding and confirm consent explicitly — all behaviours that sit on OSCE mark sheets.
