The USMLE Step 3 is the final exam in the United States Medical Licensing Examination sequence, and it is the one that decides whether you can be licensed to practise medicine in the US without supervision. If you trained outside the United States, you can only apply once you have passed Step 1 and Step 2 CK and hold a valid, unexpired ECFMG Certificate. Since January 2026 you register through the Federation of State Medical Boards rather than ECFMG, the 2026 application fee is $955, and the exam runs over two separate test days.

The USMLE Step 3 is the last of the three Step exams and the final assessment before a state medical board can license you for independent practice. The USMLE program describes its content as reflecting "a data-based model of generalist medical practice in the United States" — in other words, the clinical situations a general, as-yet undifferentiated physician would meet, with a deliberate weighting towards outpatient care.
That framing matters for how you study. Step 1 asked what a disease is. Step 2 CK asked what you would do. Step 3 asks what you would do next week, next month, and when the patient calls back — plus the biostatistics, ethics, patient-safety and systems questions that come with running your own clinic list.

You need three things at once to be eligible for the USMLE Step 3. First, passing scores on Step 1 and Step 2 CK. Second, the equivalent of the MD degree from a medical school outside the US that is listed in the World Directory of Medical Schools as meeting ECFMG eligibility requirements. Third — and this is the one that catches people out — you must have obtained ECFMG Certification and hold a valid and unexpired ECFMG Certificate.
There is also a strong recommendation rather than a hard rule: the USMLE program advises that applicants should have completed, or be near completion of, at least one postgraduate training year in an accredited US graduate medical education programme that meets state board licensing requirements. Read that carefully. It is guidance from the USMLE program, not a uniform national gate — and separately, each state medical board sets its own conditions for how many training years you need before it will grant a licence, and some set their own time limits on completing the Step sequence. Check the rules of the board you actually plan to license with, not a summary you read on a forum.
Step 1 and Step 2 CK are delivered at Prometric centres worldwide. The USMLE Step 3 is administered only in the United States and its territories. If you are sitting Step 1 in Delhi, Cairo or Manila today, you should plan on the assumption that the last exam will mean a flight. The USMLE Service Transition moved all exam registration, score delivery and customer service to the two USMLE co-sponsors. From 12 January 2026, USMLE services previously provided to international medical graduates by ECFMG transitioned to FSMB, which now manages all three Step exams for IMGs through the FSMB USMLE portal. From 26 January 2026, Step 3 services for students and graduates of US medical schools moved to NBME and the MyUSMLE portal.
Two things did not change about the USMLE Step 3. Eligibility requirements are untouched, and ECFMG continues to determine whether international graduates qualify for Certification — Step 1 and Step 2 CK still satisfy the medical science examination requirement for that certificate. What changed is the desk you file with.

If you had an application in flight during the switchover, note the program's own warning: incomplete applications and service requests not processed before the transition had to be resubmitted with the new organisation.
The USMLE Step 3 is a two-day exam, and the two days are not interchangeable. Day one is Foundations of Independent Practice (FIP): 232 multiple-choice items in 12 blocks of 18–20 items, 30 minutes per block, inside a test session of roughly seven hours that includes 55 minutes of break time and an optional five-minute tutorial. This day carries the foundational science, biostatistics and epidemiology, interpretation of the medical literature, communication, medical ethics, systems-based practice and patient safety content.
Day two is Advanced Clinical Medicine (ACM) and runs about nine hours: 180 multiple-choice items in 9 blocks of 20, again 30 minutes per block, plus a six-minute tutorial for the computer-based case simulations and then 13 to 14 case simulations, each allotted a maximum of 10 or 20 minutes of real time. A minimum of 45 minutes of break time is available.
Those case simulations are the part of the USMLE Step 3 with no equivalent in the earlier exams. You type orders in free text, advance a simulated clock, and the patient responds. Candidates routinely underestimate them because they are ungraded in most question banks — build them into your practice schedule early rather than in the last fortnight.
The application fee is $955 for three-month eligibility periods that end in 2026, and FSMB has published the same figure for eligibility periods ending in 2027. For comparison, Step 1 and Step 2 CK are $695 each. The first eligibility period at the 2026 rate ran from 1 November 2025 to 31 January 2026; the last runs from 1 October 2026 to 31 December 2026.
Budget beyond the USMLE Step 3 fee itself. Because the exam is delivered only in the US, an international candidate who is not already there is paying for flights, several nights of accommodation and two days of not working, on top of question banks. Application fees are non-refundable and are not transferable from one eligibility period to another, so pick your three-month window when you genuinely expect to be ready.

Results are reported on a three-digit scale, and the minimum passing score for the USMLE Step 3 is 200 for exams administered on or after 1 January 2024. Step 2 CK sits at 218 (from 1 July 2025) and Step 1 is reported as pass/fail only, with a passing standard of 196 on the three-digit scale. The USMLE Management Committee reviews each component roughly every four years and can adjust the standard without prior notice.
Score turnaround is usually within four weeks of your test date, but the program asks you to allow at least eight weeks before assuming something has gone wrong. That gap is worth planning around if a programme director or a state board is waiting on the result.
In 2025, 14,304 examinees from non-US medical schools took the USMLE Step 3. Of the 12,480 first-time takers, 88% passed. Of the 1,824 repeaters, 63% passed. For context, examinees from US MD-granting schools passed at 96% overall in the same year. The gap between a first attempt and a repeat attempt is the number to take seriously: this is an exam where preparation before the first sitting is far cheaper than a second registration.
Step 1 and Step 2 CK are administered worldwide across roughly 335 US and Canadian Prometric test centres and about 100 international centres. Step 3 is administered only in the United States and its territories. There is no workaround: to sit the USMLE Step 3, you travel.
What that trip requires depends entirely on your nationality and your current immigration status, and no blog post can answer it for you — check with the US embassy or consulate that serves your country well before you book a three-month eligibility period. For most international applicants the honest USMLE Step 3 sequencing is: ECFMG Certification first, then residency applications, then Step 3 during or after the first postgraduate year. A Step 3 pass on your record before Match season can help — some programmes read it as evidence you will clear licensing hurdles on time, and it is a prerequisite for certain visa sponsorship routes — but it is rarely the thing that gets you an interview. Clinical experience, letters and Step 2 CK carry more weight there.
