NRMP Registration for IMGs: 2027 Match Dates and Rules

NRMP registration is the separate sign-up that makes you an official participant in the Main Residency Match, and for the 2027 Match it opens on 15 September 2026 at 12 p.m. ET. Submitting your ERAS application does not do it for you. The NRMP (National Resident Matching Program) runs the algorithm that pairs applicants with residency positions, and it only sees you once you have created an account in its Registration, Ranking, and Results (R3) system and paid the fee. For an IMG, an international medical graduate whose basic medical degree comes from outside the United States, NRMP registration also triggers a weekly ECFMG check that decides whether you may rank programs at all.

What Is NRMP Registration and Who Has to Do It?

NRMP registration is the act of creating an account in the NRMP's R3 system, agreeing to the Match Participation Agreement and paying the Match fee. Anyone who wants a position through the Main Residency Match must do it, including IMGs, US MD and DO seniors, and previous graduates applying as independent applicants.

Plenty of first-time applicants assume one application covers everything. It does not. ERAS (the Electronic Residency Application Service, run by the AAMC) is the postal service: it carries your documents to programs. The NRMP is the referee: it collects your rank order list and each program's list, runs the algorithm and announces the result. The NRMP's calendar spells it out, telling registrants they "must register for both the NRMP and the application service or process required by the program".

One more trap: registering does not make you eligible. The NRMP requires that you "be able to enter graduate medical education (GME) on July 1 in the year of the Match" and meet all Accreditation Council for Graduate Medical Education (ACGME) entry requirements. Registration is paperwork. Eligibility is a separate test you pass by March.

When Does NRMP Registration Open for the 2027 Match?

Registration opens on 15 September 2026 at 12 p.m. ET, when the R3 system starts accepting new applicant accounts. Standard registration then stays open until 29 January 2027 at 11:59 p.m. ET. After that you can still register late, but the NRMP charges a $50 additional fee on top of the standard $85.

That mid-September opening sits close to the ERAS calendar on purpose. MyERAS submissions open on 2 September 2026 and programs start reviewing on 23 September 2026, so NRMP registration slots neatly between the two. Since 1 June 2026 you have also been able to browse the NRMP Program Directory, which the NRMP describes as covering "over 6,000 PGY-1 and PGY-2 programs".

Key 2027 Match Dates Every IMG Should Diarise

Here is the applicant calendar in one place, taken from the NRMP's published 2027 dates. Copy it into your phone, because two of these deadlines are absolute and the NRMP does not reopen them.

Notice how much lands on 3 March 2027. The NRMP's calendar stacks the rank order list certification deadline, the applicant late registration deadline, the applicant withdrawal deadline and the "IMG applicant deadline to meet ECFMG verification requirements" on the same evening. If you treat only one date as sacred, make it that one.

How Do You Register for the Match Step by Step?

Registration itself takes under half an hour if your details are ready. You create an R3 account, accept the Match Participation Agreement, enter your AAMC ID, pay by credit card in US dollars, and then wait for the NRMP to confirm your eligibility with ECFMG. Everything after that is ranking, not registering.

1. Wait for 15 September 2026, 12 p.m. ET, then open the NRMP's R3 login page and create your applicant account.
2. Select the 2027 Main Residency Match and choose your applicant type. Graduates of international medical schools register as independent applicants, not sponsored students.
3. Enter your AAMC ID exactly as it appears in MyERAS. It is the key that lets the NRMP and ECFMG line your records up.
4. Read and accept the Match Participation Agreement. It is a contract, not a checkbox, and the next section explains what you are agreeing to.
5. Pay the $85 standard registration fee by card through R3. The NRMP takes US dollars by card only, so check your card's international limit first.
6. Confirm your status inside R3 and diarise 29 January 2027 and 3 March 2027. Then leave the system alone until ranking opens.

Do You Need to Be ECFMG Certified Before You Register?

No. ECFMG (the Educational Commission for Foreign Medical Graduates, part of Intealth) states it directly: "You do not need to be ECFMG Certified in order to participate in the Match." What you do need is for ECFMG to verify to the NRMP that you have completed the required examination and skills components before the rank order list deadline.

The mechanism is quietly clever. Once a week after registration opens, the NRMP sends ECFMG its list of registered IMG applicants, and ECFMG replies with a yes or no on whether each one has completed the medical science examination, clinical skills and communication skills requirements. Your NRMP registration is what puts you on that weekly list.

For the 2027 Match, the NRMP notes that because USMLE Step 2 CS was discontinued, ECFMG has identified pathways that let qualified IMGs satisfy the clinical and communication skills requirement. Satisfy a pathway and hold passing USMLE Step 1 and Step 2 CK results valid for certification, and ECFMG will verify your exam requirements to the NRMP. The consequence is blunt: IMGs who have not met those requirements by the rank order list deadline cannot participate in the Match. Full certification is still required before you start training. Registration and matching simply come first.

What Does the Match Participation Agreement Commit You To?

The Match Participation Agreement is the contract you accept during NRMP registration, and it turns a matched result into a binding commitment for both you and the program. Neither side can walk away from it without a waiver or deferral granted by the NRMP. Positions accepted during SOAP carry exactly the same weight.

Read it slowly before you click accept. Three consequences follow:

• You cannot withdraw after 3 March 2027. The withdrawal deadline shares the 9 p.m. slot with rank order list certification. Once it passes, you are in.
• Ranking a program means you are willing to go there. The algorithm does not ask twice, so never rank a program you would refuse.
• Unpaid fees can end your season. The NRMP states that applicants with outstanding fees cannot enter rank order lists or be ranked, and applicants withdrawn for unpaid fees lose access to the List of Unfilled Programs and cannot take part in SOAP.

SOAP, the Supplemental Offer and Acceptance Program, is the Match Week process where unmatched and partially matched applicants apply to unfilled positions. ECFMG lists four things an international applicant needs to be SOAP-eligible: MyERAS 2027 registration, NRMP registration for the 2027 Main Residency Match, being fully or partially unmatched, and meeting all Match eligibility requirements. Registration Fees and the Price of Being Late

Standard NRMP registration is $85, covering up to 20 unique program codes on your primary rank order list plus 20 across all supplemental lists. Beyond that the extras stack up: $30 per code ranked above 20 up to 300 ranks, $50 for registering after 29 January 2027, $45 per partner to enter as a couple, and $50 to $200 more for lists of 100 or more ranks.

Every one of those fees is non-refundable. The extra rank fee and the list-length fee are charged the moment you certify your list, and the NRMP will not let you shorten a certified list to claw the money back. IMG Numbers in the Match and Where an Elective Abroad Fits In

It helps to see the scale of what you are registering for. Intealth's summary of the 2026 Main Residency Match, drawn from the NRMP's Advance Data Tables, shows IMG participation climbing while the matched share has hovered around 60 percent for five straight years.

Roughly four in ten IMGs who register do not match. That is not a reason to panic in September, but it is a reason to build a serious application rather than a hopeful one, and to have a plan for SOAP before Match Week.

Where does a clinical placement abroad sit in this? To one side of it, honestly. An elective in Sri Lanka, Nepal, Bali or Zanzibar is not US clinical experience, and no NRMP or ECFMG rule counts it as such, a distinction we set out in US clinical experience for IMGs.
